No Information From Children Under Age 13

Security Of Your Personal Information

SuperCare Health uses industry-standard safeguards to protect your personal information from unauthorized access, use, or disclosure. SuperCare Health safeguards the personally identifiable information provided on computer servers in a controlled, secure environment, protected from unauthorized access, use, or disclosure. When personal information (such as a SuperCare Health Account ID) is transmitted to other websites, it is protected through encryption, such as the Secure Socket Layer (SSL) protocol.

Additionally, we maintain physical, electronic, and procedural safeguards to help prevent unauthorized access to personal information. We update and test our technology periodically to improve these protections and ensure personal information’s integrity. While we use these precautions to safeguard your information, we cannot guarantee the security of the networks, systems, servers, devices, and databases we operate or that are operated on our behalf.

You have the right to inspect and obtain copies of your health information maintained by SuperCare.

When legally available and technically feasible, you may request electronic copies of your records. We may charge a reasonable, cost-based fee for copies.

Right to Request Corrections

You may request that we amend information you believe is inaccurate or incomplete.

We may deny certain requests as permitted by law, but we will explain the reason in writing.

Right to Request Restrictions

You may request restrictions on certain uses or disclosures of your PHI.

While we are not required to agree to most requested restrictions, we will comply when required by law.

Right to Restrict Disclosure to a Health Plan

If you pay for an item or service completely out of pocket, you may request that we not disclose information about that item or service to your health plan for payment or healthcare operations purposes.

We will comply with such requests when required by law.

Right to Confidential Communications

You may request that we communicate with you through alternative means or at alternative locations.

We will accommodate reasonable requests.

Right to an Accounting of Disclosures

You may request a list of certain disclosures of your PHI made by SuperCare during the six years prior to the date of your request. This accounting will not include disclosures made for treatment, payment, or healthcare operations, disclosures made to you or with your written authorization, or other disclosures excluded by law. We will provide one accounting free of charge in any 12-month period, and we may charge a reasonable, cost-based fee for additional requests within that period.

Right to Receive a Paper Copy of This Notice

You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.

BREACH NOTIFICATION

You have the right to be notified if a breach occurs involving your unsecured protected health information and the breach requires notification under applicable federal or state law.
