SuperCare Health and DaVita HealthCare Partners conducting iBreathe clinical study investigating program to reduce COPD hospital readmissions


DOWNEY, CA – SuperCare Health – a high-touch, high-tech chronic care management company specializing in respiratory diseases – has initiated a clinical study with DaVita HealthCare Partners to evaluate the impact of their Healthcare At Home iBreathe Program on patient hospital readmissions for chronic obstructive pulmonary disease (COPD) exacerbations. The Centers for Medicare & Medicaid Services reports that, on average, 1 in 5 patients are readmitted to the hospital within 30 days of a prior hospitalization for a COPD exacerbation with costs associated with rehospitalizations at $10,900 for each readmission. The iBreathe study is currently enrolling patients within DaVita’s HealthCare Partners division who have COPD, are over 50 years of age, and have been discharged from the hospital with a COPD diagnosis.

The iBreathe study is based on SuperCare Health’s Healthcare at Home program that provides COPD patients with a comprehensive management program involving an interdisciplinary care team of respiratory therapists, nurses, pharmacists, social workers and dietitians. Patients enrolled in the program receive a personalized program led by a respiratory therapist that provides an iPad, diagnostics, medication treatment optimization, medication management tools, education and support to help them better self-manage their COPD.


On the first day, a respiratory therapist conducts a thorough clinical assessment in the patient’s home, and over the course of the 30-day program, conducts scheduled telehealth video appointments with the patient to assess clinical status, encourage treatment and medication compliance, further educate, provide social services and identify any issues that may be keeping the patient from successful self-management.

The iBreathe study is an IRB-approved, randomized clinical trial that will enroll 200 HCP members diagnosed with COPD. Patients over 50 years old who are admitted to the hospital for a COPD-related exacerbation are eligible for the clinical trial and will be randomly assigned to receive either standard care or enhanced care management through SuperCare Health’s iBreathe program.
